To complete the registration process, you will need to fill out all required fields that are marked with an "*".  Once you have filled out all fields that you can, remember the more information the better for your child's health and safety, click the registrar button at the bottom of the form.  If this is your child's first time playing with FAST, a copy of the birth certificate is required and can be included as part of the registration if you have an electronic copy.  If you do not have an electronic copy, you will have to make arrangements as soon as possible with the FAST board.  Also your registration will not be complete without it.  A confirmation email with the information that you use on this form will me sent to you.  Once you have clicked the register button, you will be directed to a payment page. YOU MUST PUT YOUR CHILD'S NAME IN THE INSTRUCTIONS SECTION OF PAYPAL.  Once you have made your payment, your child will be registered.
